Adoption Application

We're so excited that you want to adopt from Fawn's Small Dog Rescue!

Important! Due to the length of the Adoption Application we recommend using a computer or tablet instead of a smartphone.

Before you begin...

You will need the following information to complete your application. We recommend having it readily available to make your application easier to fill out.

  • Current and prior veterinary clinic/hospital information including their name, address, and phone number.

  • Information on your current and prior pets medical history.

  • One or more personal references and their phone numbers.

  • Renters must include their landlord or property management company's name and phone number

Adoptable Dogs

Who are you interested in adopting? Select one or more dogs.

This is important if the dog you're applying for has specific needs for their ideal home - especially if you don't meet every requirement on their list.

As a volunteer-run organization, it takes time and effort to review applications. Confirm the following:

This application process includes a basic background check, virtual or in person home visit, veterinary reference check, and personal reference check.

Fawn's Adoption Application processes includes a vet check. Where we confirm your current or previous animals are spayed/neutered and up to date on their vaccines and monthly preventatives.
